Services paid at full benefit percentage if obtained at Union Hospital, Sullivan County Hospital, Union Hospital Clinton, Crawford Memorial or Greene County Hospital. Otherwise services will be 50% reimbursed. Exceptions allowed for emergencies, medical necessities or procedures not done in these facilities. |
